All forms must be turned in for every athlete. Each requires parent signatures. Additional forms may be required by individual coaches.

This form outlines the commitment students make to participate in sports: to maintain academic eligibility, to fully commit to their teams and to uphold a standard of character.

This form is signed by a physician stating that the athlete is medically cleared to play sports.

This form gives permission to the team/coach to seek medical treatment in the case of an emergency.
